Download the form below, list every college your child is applying to along with the application plan for each one (ED, ED II, EA, Restrictive/Single-Choice EA, Regular Decision, or Rolling), sign it, and send it back to us. This is the only way we know where to send your financial aid applications.

The form is fillable. Type your list right into it, then email it to [email protected] or fax it to 516-908-4418. Page 1 holds 16 colleges. If you need more room, use page 2 and send both pages. Please return it no later than 72 BUSINESS HOURS before your earliest deadline.
